Physician Competency Checklist (optional review only)
A. Procedure
1. Follow proper procedure for patient identification and specimen labeling as required by JCAHO.
2. Testing procedure:
- Add 3 drops of Reagent one and 3 drops of Reagent two in the extraction tube. The solution should turn yellow.
- Add specimen swab immediately and vigorously mix the solution by rotating the swab forcefully against the side of the tube and least ten times.
- LET STAND ONE MINUTE
- Express as much liquid as possible from the swab by squeezing the sides of the tube as the swab is withdrawn. (Not squeezing enough liquid out of the swab can cause an invalid test.)
- Place absorbent end of the test stick into the liquid
- Background should clear and red control line should appear
- Read test result at 5 minutes per manufacturer
3. Document Results
- Date, time, and initials of person performing test on log
- Internal quality control results
- Patient test results (including electronic record if applicabl
B. Quality Control
1. Internal Control
- Color change from pink to light yellow when Reagent 2 in added to Reagent 1
- Red control line
- Clear background
2. External Controls
- A positive and negative control must be tested at a minimum with each new lot number, each new shipment, and with each newly trained operator.
- Document results and any necessary corrective action for internal or external quality control failures.
C. Infection Control
- Use of gloves when handling samples and controls
- Proper disposal of all biohazardous supplies and materials.
